本病无明显季节性,夏秋季略多。可呈暴发流行。
临床表现为较重的腹泻,稀水样便,每日3~30次不等。常有呼吸道症状。如咽炎、鼻炎、咳嗽等,发热及呕吐较轻,可有不同程度的脱水征。病程8~12天。多数患儿病后5~7个月内对蔗糖不耐受,并可伴有吸收不良。
诊断根据免疫电镜检测粪便中肠腺病毒颗粒或用免疫荧光法等检测粪便中肠腺病毒抗原。
诊断说明
主要检测粪便中腺病毒。普通电镜观察即能明确腺病毒存在,用免疫电镜可分型。粪便原液用血凝抑制试验或ELISA可测得腺病毒抗原。应用聚丙烯酰胺凝胶电泳法亦可测得粪便中腺病毒,阳性率高于电镜。
由于肠腺病毒不能进行常规细胞培养,因此采用DNA限制性内切酶检测法以测定腺病毒的特异基因,但方法复杂,且需多量病毒存在,故虽特异性强,但难以开展。
治疗说明
腺病毒肠炎病情不重,一般不治自愈。主要是对症治疗及必要的支持疗法。有轻度失水可口服补液,中或重度失水宜静脉补液。
腺病毒胃肠炎的zui小发病年龄为1月,绝数发生于3岁以下的婴幼儿。Ad-40主要感染1岁左右婴儿,Ad-41则感染稍大的幼儿,全年均可发病,但腺病毒性胃肠炎一般呈散发性。
腺病毒胃肠炎的潜伏期约10天,主要表现为腹泻,量少或多,呈水样便或稀便,少数可排出粘液,病程4~8天,常伴呕吐。部分患者有呼吸道症状,少数患者发热,病程多自限,排毒时间约1周。某些患者失水较严重,个别重度失水者可死亡。
由于对一病毒的致病作用不够重视,尚无常规细胞培养。诊断本病虽可采用DNA限制性内切酶检测法,但此法有一定局限性,不易推广。有人认为采用聚丙烯胺凝胶电泳法既能检测Ad,又可检测粪便中轮状病毒,敏感度高于电镜。
对本病的治疗主要是对症治疗,可口服世界卫生组织推荐的口服补液盐,必要时给予静脉补液。
腺病毒(adenovirus)是一种没有包膜的直径为70~90 nm的颗粒,由252个壳粒呈廿面体排列构成。每个壳粒的直径为7~9 nm。衣壳里是线状双链DNA分子,约含4.7kb,两端各有长约100 bp的反向重复序列。由于每条DNA链的5'端同相对分子质量为55X103Da的蛋白质分子共价结合,可以出现双链DNA的环状结构。
